Community Nursing Team

The Community Nurses
The community nursing team can be contacted at the surgery and there is no need for patients to be referred by a doctor. If you, a relative or friend, need the services of the nursing team at home, please contact the nurses through reception at the surgery during opening hours and arrangements can be made to visit. Services provided by the team are home treatments, health promotion, advice, elderly assessments, falls assessments and presentations, referrals to other medical professionals when needed.

Health Visitor
The health visitor is a registered general nurse with specialist training in family health needs. The health visitor provides house visits for antenatal and after the birth of the baby. 

In addition, support is available for contraception, bereavement, smoking cessation, relationship issues, depression and benefit claims to name but a few. The health visitor also provides ongoing support and coordination of services for a child with special needs. She can be contacted through reception.

PCN Team

Primary care networks (PCNs) are groups of GP practices working closely together with other community, mental health and social care staff to improve services for local people.

PCNs will be responsible for delivering joined-up health and care services through multi-professional teams to patients in the community.

We are part of the West Norfolk Coastal PCN. We share this team with local practices within our coastal PCN.
